What the paper asked

For picking out adults at cardiometabolic risk, which works best: body mass index (BMI), waist circumference, or waist-to-height ratio?

What was found

  • Across all outcomes together, waist-to-height ratio discriminated significantly better than BMI: roughly 4–5% better (P < 0.01).
  • Waist circumference alone also beat BMI, but by less: about 3% (P < 0.05).
  • Compared within each study, waist-to-height ratio also beat waist circumference itself — for diabetes, hypertension, cardiovascular disease and all outcomes together, in men and in women (P < 0.005).
  • Outcomes covered: hypertension, type 2 diabetes, dyslipidaemia, metabolic syndrome and cardiovascular outcomes.

What does not follow from it

  • This is a screening result, not a treatment one: the studies used ROC curves to measure how well each index separates people at risk from people who are not. Whether lowering the ratio lowers the risk is a different question, and this paper did not answer it.
  • The differences are small — a few per cent. All three indices are close; waist-to-height ratio is better, not in a different league.
  • The pooled studies are observational, not trials. They show the ratio travels with risk, not that it causes it.

What it means for someone here

In this clinic's 3D body scan, waist-to-height ratio is reported straight from the scan and needs no equation, no sex and no weight. This paper is why it sits beside the body-fat figure and ahead of BMI: its simplicity is not a weakness. Our report reads it against a boundary of 0.5. One caveat: if the scan cannot tell the narrowest point of the waist from the roughness of its own surface, the waist — and this ratio with it — is not reported at all, rather than filled with a guess.
