What do the tools that measure posture actually tell us about back and neck pain?

What was found

  • What these tools do well: construct validity and inter- and intra-rater reliability. They measure what they claim to, and they measure it again the same way.
  • What they do least: sensitivity to clinical change and predictive validity. Whether a change in the number tracks the person getting better is unclear, and so is whether the number predicts anything.
  • The authors note that most of these tools still rest on a postural-structural-biomechanical model of pain rather than the biopsychosocial model that is now better accepted.

What does not follow from it

  • This is a scoping review: a map of the literature, not a meta-analysis of effect. No effect size comes out of it.
  • The search ran to August 2023.

What it means for someone here

A corrective exercise clinic does not usually quote this paper, because it is not comfortable. Its point: posture can be measured precisely, but the link between the number and the pain is not simple, and these tools were not built to predict. That is why this clinic's report does not call a number a diagnosis and never says "this number is causing your pain" — it says what was measured, at what grade, and what was found beside it. The clinical decision belongs to whoever is looking at the patient.
